The Appointment
What to bring
- Photo ID - Passport or Driving Licence. Must be in date and originals only
- Driving Medical Form. For D4 you can obtain from the DVLA and for Taxi Medicals, from the local licensing authority. Please fill your details on the form before the appointment
- Payment - Card or Cash
- Glasses for distance vision (not reading) bring them with you and a copy of your most recent optician's report. Contact lens users should wear their glasses for the medical
The Examination
The examination will check
- Vision
- Blood Pressure
- Weight and Height (D4 Medicals)
Issues that can cause a fail / delay to passing
- Blood pressure that is poorly controlled
- Vision that does not meet the minimum standard
- Diabetes that is poorly controlled
- Other disqualifying conditions
- Failure to bring ID or Driving medical form
