Medical History

You need to obtain a summary of your medical record from your GP surgery

The request for this should usually be in writing - but they may accept via phone or econsult.

Your surgery may issue a short summary with just the last 3 consultations only. This will usually not be enough (see below)

It is best to obtain in electronic format that you can forward via secure email when booking.

The summary should at the very least contain:


  • List of medical conditions - current and past
  • List of current medications 
  • At least 3 year's consultations from when last seen / spoken to, including any hospital / clinic correspondence in that time also
  • If you take blood pressure medication, the summary will need to have the last 3 readings on it.
  • If you are currently under a hospital specialist then you need to obtain your most recent clinic letter
  • If you have had any heart investigations or treatment, you need to obtain the letters outlining what has been done and when 

              Eg: ECG, angiogram, myocardial perfusion scan etc 

  • If you are diabetic, you need to show most recent 

              - Hba1c bloods 

              - foot and eye screening 

              - Insulin treated - 6 weeks of continuous blood glucose readings on a    

                memory meter

  • If you have Obstructive Sleep Apnoea, you need confirmation you are compliant with treatments and not suffering with excess daytime sleepiness 

Why do you need to obtain this information?

All of the above are taken from the DVLA Group 2 Medical Guidelines 

ie: The D4 form for HGV, PSV etc


Taxi licensing authorities usually require the same but may vary.


We will check all the relevant information is received to ensure nothing additional is required at your appointment 


What conditions can affect the ability to pass a medical?

Various medical conditions that can affect a commercial driver’s ability to drive safely, for example:


  • Blackouts or fainting
  • Vision problems
  • Epilepsy
  • Heart disease
  • High Blood Pressure (poorly controlled)
  • Diabetes (poorly controlled)
  • Sleep disorders
  • Neurological disorders
  • Psychiatric disorders
  • Age-related decline
